Recommendation Request Questionnaire, написанный одним из учителей английского, мой ребенок не брал у него рекомендацию. Учитель просит освежить его память об этом конкретном ученике и дать ему какой-то материал, который он может использовать в письме.
Write several paragraphs telling what you remember about the class and describe any or all of the following topics. The more details you provide, the better. Discuss a specific project, class discussion or presentation in which you were a significant participant. How and when did you come in for help? How did you work through challenging topics? What was the most important thing you took away from this class? What was the most memorable learning experience? Describe the personal and/or academic qualities or strengths you would like to have emphasized in the letter. Use specific examples from class experiences where these strengths were shown.
Were there any personal, family or other circumstances that affected your performance in this class? Discuss how you overcame these obstacles and any marked improvements in your performance as a result.

Предполагается, что ученик берет рекомендации у учителей предметов, которые так или иначе связаны с потенциальной специальностью. Мой брал у учителей математики и физики, подавая на инженерные специальности, но те писали все сами, без подсказок.
Канслеры просили еще более подробный brag sheet, потому что школа огромная, на каждого канслера по 100 синиоров. Но я не могу сейчас найти вопросы, они были все в онлайновой форме, а доступа туда уже нет :(. Там было что-то более общее - чем ребенок увлекается, в чем преуспел, что не удалось. Каждая часть application должна логически дополнять остальные, "писать картину", чтоб за всеми этими формальными документами проступал реальный человек. Из этого и исходите.
